cultured from a sputum sample. He mentions that his stools are really hard to
flush away.
2. A 50y Algerian chef
presents to outpatients complaining of bloating, abdominal pain and fatigue
that is particularly bad after eating couscous. Duodenal biopsy shows
intraepithelial lymphocytes and villous atrophy.
3. A 45y business man consults his GP complaining of arthralgia, fever,
sweats and weight loss. Examination reveals lymphadenopathy. A jejunal biopsy
reveals stunted villi and PAS positive macrophages. What complication is he at
risk of?
4. A familiar alcoholic presents to A&E with epigastric pain that
‘bores’ through to his back. His clothes seem baggier than usual and on closer
inspection he is noted to have large areas of bruising.
5. A 50y woman presents to her GP with prolonged morning stiffness and
swelling of some of the small joints in both hands. She also complains that her
face and ankles seem puffy and wonders if this is related. Blood tests reveal
raised GGT and Alkaline Phosphatase.
